This image was altered in photoshop. The original image had a charcoal filter effect put on it giving it the scratchy appearance it has. Also it looks like a sepia tone was put on the picture and the contrast was altered making the shadow as dark as it is. It seems as if the original image maybe had a blur tool used on it to blur the background the way it did.
This image was definitely created using photoshop. It is an extremely unrealistic picture that is so well made that it almost seems real. Different body parts were individually cut out and pasted into the picture and in order to make them seem attached to her head a smudge tool was probably used to make it not cut out and like a more smooth transition from her head to her body parts. Also the image was put onto a picture of a web which is the background.
